Fees for review of ethics applications funded by a for-profit entity (i.e., pharmaceutical/medical device company).

Penalty for Lapsed Approvals	$250
Amendments requiring Full Board Review	$500

The investigators remain responsible for communicating fee requirements to the sponsor. Payment is expected within 30 days of receipt of the invoice.
